Please submit this form no later than a week before Yahrzeit date, therefore FPJC can add Yahrzeit to our weekly Daf.  

IF YOU ARE DESIGNATING A TORAH STUDY AS A NEDIV YOM, OR ANY OF THE BELOW, IT IS URGENT TO RETURN THIS FORM WITH YOUR DONATION INDICATING YOUR CHOICE.

Yahzeit

The Yahrzeit candle should be lit the evening before Yahrzeit date.  It is proper for those observing a Yahrzeit to recite the Mourner's Kaddish at Services.  If you would like to come for a minyan, please call in advance for our minyan schedule.

May we respectfully call your attention at this time to appropraite ways in which your departed may be memorializes?

1. A contribution to the Scholarship Fund of the Jewish Center.
2. Nediv Yom- Your donation of $36 will designate the Torah Study and prayers of that day in memory of ypur loved ones.
3. You may sponsor a Seudah Shlishit on Shabbat for $72 or co-sponsor a Kiddush for $400.
4. Bronze Memorial Tablet. Adjoining lamps are lit for YAHRZEIT and for YIZKOR 
(Non-members $550, FPJC members $500).
5. Arrangements can be made for Perpetual Kaddish ($250) and/or Perpetual Nediv Yom ($500).
